Module 4 — Bilateral Stimulation and Dual Attention

Bilateral stimulation is rhythmic left-right stimulation of the body or senses, eyes tracking a hand sweeping side to side, taps alternating between the left and right hands, tones switching between the left and right ears, delivered while a person's attention rests on something internal. It is the strange-looking part of EMDR, the part that made skeptics roll their eyes for decades, and the part everyone asks about first. In the field it goes by its initials, BLS, and this lesson opens it up completely: what it is, the three forms it takes, how it is dosed, and the genuinely fascinating scientific detective story about why moving the eyes back and forth would help a painful memory lose its grip.


The word "bilateral" is doing the important work in the name. Bi, two; lateral, sides. Whatever form it takes, the stimulation alternates rhythmically between the left side and the right side, left-right, left-right, like a metronome the body can feel. And the second half of the definition matters just as much as the first: BLS is not administered to a person sitting idle. It happens while attention is held on internal material, a memory, a feeling, an image, a belief. The pairing is the point. The rhythm alone is just rhythm. The rhythm plus focused inner attention is where EMDR happens.



THE THREE CHANNELS OF BLS


Bilateral stimulation can enter through three different senses, and a trained clinician chooses among them the way a craftsperson chooses tools: by fit, not by rank.


  • Eye Movements. 

    The original channel, the one discovered on Francine Shapiro's famous walk, and by far the most researched. The therapist holds up two fingers and sweeps them horizontally across the person's field of vision, and the person tracks the movement with their eyes while their head stays still. Some clinicians use a light bar, a device with a dot of light gliding side to side, which does the same job with more precise control over speed. Eye movements are typically the first choice because the bulk of the scientific evidence was built on them, and because there are indications that keeping the eyes busy has effects the other channels may deliver less strongly, a thread picked up in the theory section below.


  • Tactile Tapping. 

    The stimulation moves to the sense of touch: the therapist taps alternately on the person's hands or knees, or the person holds small devices called pulsers, one in each palm, that buzz in alternation. Tapping earns its place for solid practical reasons. Some people find sweeping their eyes back and forth distracting or physically uncomfortable, and some process better with their eyes closed, where imagery comes easier and the therapy room fades back. Someone deep in a visual memory often stays with it more fully when nothing is asking their eyes to work.


  • Auditory Tones. 

    Through headphones, a soft tone alternates between the left ear and the right ear. Auditory BLS is the quietest of the three and often the choice when the other channels do not fit: for people with visual impairments or eye conditions that make tracking painful, for those who dislike being touched or find the pulsers distracting, or simply for those who settle best with their eyes closed and their hands free.


Nothing sacred attaches to any single channel, and clinicians sometimes switch mid-treatment if one is not serving. What every channel preserves is the signature: alternating, left-right, rhythmic, while attention rests inward. That is the constant. The doorway it enters through is negotiable.



WHAT A SET IS


BLS is not delivered as one long continuous stream. It comes in units, and the unit has a name worth owning, because every description of EMDR from here forward is built on it: the set.


A set is one continuous run of bilateral stimulation, commonly somewhere in the range of twenty to forty seconds, though clinicians adjust the length to the person and the moment. Then the stimulation stops. There is a pause, a breath, and a brief check-in, often nothing more than the therapist asking what the person is noticing now. Then, typically, another set begins.


That pulse, set, pause, notice, set again, is the heartbeat of EMDR sessions. The rhythm exists for a reason that says something about the whole method: the pauses are not rests between the "real work." They are where the person surfaces, notices what has shifted, and lets the next set begin from a new place. What the therapist does with those check-ins, and where the sets lead, is the territory of the desensitization lesson later in this course. For now, the term is what matters: when EMDR people say "a few more sets," this is the unit they are counting in.



FAST BLS AND SLOW BLS: TWO DIFFERENT JOBS


Here is the distinction this entire course quietly rests on, and it is beautifully simple: the speed of the stimulation changes what it does.


Fast BLS taxes and loosens. 

Rapid stimulation, quick eye sweeps, brisk alternating taps, demands something from the brain. It occupies attention, works the mental machinery, and seems to loosen the grip of whatever material is being held in mind. This is the speed used for processing distressing memories: the demanding rhythm is part of what helps frozen material start to move. Picture Jordan, mid-session with his therapist, holding a hard memory while his eyes track fingers moving quickly. The pace is not gentle, and it is not meant to be. It is doing work.


Slow BLS soothes and anchors. 

Slow the same stimulation down, unhurried taps, a gentle lazy rhythm, and its character transforms. Instead of taxing, it calms. Slow bilateral rhythm settles the nervous system and is used to deepen and strengthen good states: calm, safety, confidence. Picture Jordan's therapist, later in that same session, guiding him to rest in a peaceful feeling while she taps slowly on his knees, left, right, left, right, at about the pace of a slow heartbeat. Same channel, same alternation, opposite job.


If the pairing feels familiar, it should: fast rhythm activates and works, slow rhythm settles and soothes, which is roughly the difference between a workout and being rocked. And this distinction is not trivia. It draws the most important practical line in this course. Slow BLS on calm or positive material is gentle, stabilizing, and safe in a person's own hands; it powers the self-usable skills taught in the resourcing lesson ahead, including a famous self-applied technique that gets its full teaching there. Fast BLS on distressing material is the engine of memory processing, and it belongs in the therapy room, where a trained clinician is tracking what opens up. When this course says some EMDR skills are yours and some are clinician-guided, this distinction, speed and material, is the reason why.



DUAL ATTENTION: ONE FOOT IN THE PAST, ONE IN THE PRESENT


Beneath the taps and the tones sits the deeper engine, and it has a name: dual attention.

Notice what BLS forces a person to do. Part of their attention rests on internal material, the memory, the feeling. But another part must stay in the present room, because something in the present keeps demanding it: eyes have to track the moving fingers, skin has to register the alternating taps, ears have to follow the tones. The person cannot fully leave the room, because the stimulation keeps knocking. Attention is split, deliberately, in two: one foot in the past, one foot in the present. Shapiro considered this so central that for a time she referred to the stimulation itself as dual attention stimulus.


Why engineer a split like that? Because of the difference between remembering something and reliving it. The past-is-present principle from the AIP lesson explains that a stuck memory can fire as though the event is happening now; a person pulled fully into that material is not remembering the worst moment of their life, they are in it, and from inside it, no processing happens, only re-experiencing. Dual attention prevents the full plunge. The present-moment anchor, the fingers, the taps, the tones, keeps part of the person planted in a safe room in the present year, so the memory can be touched without swallowing them. One foot stays on the riverbank while the other tests the water.


That anchoring is a large part of what makes approaching unbearable material bearable at all. People who have avoided a memory for decades, sensibly, because every approach meant drowning in it, find they can visit it in this split-attention state: close enough to work with, held enough not to drown. The memory is experienced as a memory, something being looked at, rather than something happening. The full machinery built around this, and what happens when the water runs high anyway, belongs to later lessons. What belongs here is the principle: BLS is not just rhythm. It is an anchor that makes the past approachable by keeping the present in the room.



WHY IT WORKS: THE LEADING THEORIES


Now for the detective story, and it deserves honest framing: EMDR's results are well established, but the mechanism, the precise reason bilateral stimulation helps, remains a live scientific question with competing theories. Two lead the field, and they are both genuinely elegant.


The working memory theory. 

The mind's active workspace, the mental desk where whatever you are currently thinking about gets held and manipulated, is called working memory, and its defining feature is that it is small. It can only hold so much at once. Now watch what EMDR does with that fact: it asks a person to hold a vivid, emotionally loud memory on that little desk, and at the same time gives the desk another job, tracking fingers, monitoring taps. The two tasks compete for the same limited space, and something has to give. What gives, according to a substantial body of laboratory research, is the memory's vividness: images recalled while the eyes are kept busy become measurably blurrier, dimmer, and less emotionally intense, and the effect appears to persist after the exercise ends, as if the memory gets re-stored in its degraded, quieter form. On this theory, EMDR works by repeatedly taxing the workspace while the memory sits on it, sanding the charge off a little more with every set. It is currently the best-supported account, and it fits neatly with people's reports that images "faded" or "moved farther away."


The orienting-response and REM-linked theory. 

The second account reaches for something older in the nervous system. When something moves in the environment, mammals reflexively orient to it, attention snaps outward, followed, when no threat appears, by a wave of settling. Rhythmic left-right stimulation may repeatedly trigger this ancient orient-then-relax cycle, telling the deep brain, over and over, that the environment is being scanned and it is safe, a state in which the nervous system loosens its grip on defensive holding. And there is a companion observation almost too poetic to be a coincidence: the rapid, rhythmic eye movements of EMDR resemble the darting eyes of REM sleep, the dreaming phase in which the brain is believed to do much of its nightly emotional processing and memory consolidation. Perhaps, this theory suggests, bilateral stimulation borrows the brain's own nightly processing state and switches it on while the person is awake and aimed at a chosen memory.


The honest summary: the working memory account has the strongest experimental support; the REM and orienting ideas remain compelling but harder to confirm; and the theories are not mutually exclusive, since a mechanism can have more than one gear. Researchers also still debate how much the eye movements add beyond the other ingredients of the therapy, a debate that has sharpened the research rather than sinking the method, because the outcome trials keep landing in EMDR's favor regardless. Somewhere between the mental desk and the dreaming brain, the full answer is still being worked out, and it is one of the more interesting open questions in clinical science.

EMDR PRACTICE: FEELING THE RHYTHM


This practice has one modest goal: to let you feel what bilateral rhythm is like in your own body. Nothing is being processed and nothing is paired with the rhythm; you learned in this lesson that the pairing is what makes BLS do therapeutic work, so this practice deliberately leaves the second half out. It is slow rhythm only, in a neutral state, purely to acquaint you with the sensation.


Find a quiet spot where you can sit comfortably for two or three minutes.


  1. Sit with both feet on the floor and your hands resting on your thighs, one on each leg.


  2. Take a breath or two and let yourself settle into a neutral, ordinary state. Nothing to summon, nothing to fix. If right now happens to be a stressed or upset moment, save this for a calmer one.


  3. Begin tapping gently and slowly, alternating: left hand, right hand, left, right. Aim for an unhurried pace, roughly one tap per second, like a slow, relaxed heartbeat.


  4. Keep the taps light. This is not drumming; it is a rhythm your body can barely be bothered by.


  5. As you tap, simply notice the sensation of alternation, attention drawn gently left, then right, then left. Notice what the rhythm feels like: its steadiness, its predictability, whatever quality it has for you.


  6. After two or three minutes, let the tapping fade out. Sit for one more breath and notice how your body feels compared to when you started. Any difference, or no difference at all, is a fine result; the goal was acquaintance, not change.


This Week


  • Once this week, notice a naturally bilateral rhythm you are already inside: walking is the obvious one, left foot, right foot. See what a busy mind does after ten minutes of that steady alternation. There may be a reason so many people say they think best on walks.


  • If you swim, cycle, knit, or drum, notice the left-right alternation hiding in it, and notice the state it tends to leave you in.
